
Library Staff Performance Evaluation Forms
This form has two parts: a self-evaluation to be completed by the employee and
an annual evaluation to be completed by the supervisor. Please see the Performance
Evaluation Procedures below for more information.
Performance Evaluation Procedures:
The performance evaluation process will occur during March and April. The process
focuses on communication between the supervisor and staff member to recognize
accomplishments and progress on goals during the past year, evaluate overall
job performance, and establish expectations and goals for the coming year.
Procedures:
- Staff members fill out the Self Evaluation form and turn it in to the supervisors
by date requested.
- Staff members work with their supervisors to schedule the performance evaluation
meeting.
- Supervisors prepare the Supervisor's Evaluation form for the staff members
keeping in mind the Library's performance criteria (see below) and incorporating
information from the Committee/Project Team Member Evaluation into the evaluation.
- Supervisors assign an overall performance rating (based on the criteria
below) to each staff member and discuss the rating and evaluation with their
Director or supervisor prior to the performance evaluation meeting with the
staff member. For staff members whose performance rates as Exemplary Service,
Supervisors/Directors will also decide which staff member(s) should receive
a merit award. Those names should be forwarded to the Director of Administrative
Services.
- Supervisors should give the performance evaluation to the staff members
to review prior to the scheduled performance evaluation meeting.
- Supervisors bring the evaluations to the meeting with the staff members.
- Supervisors and staff members come to the meeting prepared to discuss the
evaluation and reach agreement on goals for the coming year.
- Supervisors and staff members sign the supervisor's evaluation form. (Staff
members have an opportunity to include their comments on the evaluation form
prior to signing).
- Supervisors send completed forms to their Director.
- All signed evaluations will be due to the Library Administrative Services
office by the last business day in April. Directors send forms to the Library
Administrative Services office for the staff member's personnel file.
- Supervisors and staff members are encouraged to review goals throughout
the year and communicate when changes and questions arise.

Library Job Description Template
Based on the 2005 Performance Evaluation Review Committee's recommendations,
during the annual evaluation process, supervisors should review the staff member's
job description, making any revisions necessary. If staff members do not currently
have a job description, supervisors should work with their staff members to
draft and complete the job description. The goal is for all employees to have
job descriptions. These descriptions are especially important as they clue employees
as to the points that they need to address in their review. Below is a template
for these descriptions, to help supervisors and staff members across the library
system maintain consistency.
